Registration Form
Upon receiving your registration, a member of our team will send you an invoice for your deposit, and balance due. You will also receive a service contract and waiver. The contract will need to be completed and returned to 365 Spirit before your session begins. Each participant will be required to return a signed liability waiver. No one will be allowed to participate without a signed liability waiver. Waivers will be collected at the beginning of your clinic or choreography session.
